Your Medicare card is proof of your Medicare insurance. If your Medicare card was lost, stolen, or destroyed, you can ask for a replacement by using your online my Social Security account.
If you can’t or prefer not to use the online service:
You can call Medicare at 1-800-633-4227 TTY 1-877-486-2048) (was 1-800-772-1213 (TTY 1-800-325-0778)), Monday through Friday, from 7 a.m. to 7 p.m.; or
Or
You can contact your local Social Security office to make an appointment. Use this Social Security link to find you local Social Security office. (As of January, 2024 and earlier Social Security offices are now back open. On March 17, 2020, Social Security suspended face-to-face service to the public in field offices and hearings offices nationwide until further notice. However, critical services are still provided via phone, fax and online. You can use the local office lookup to obtain additional phone numbers).
New Medicare cards as of 2018 –
As of 2018, new Medicare cards are in the mail. As of 2/1/2019, all new Medicare cards have been mailed. The new Medicare cards have a new Medicare Number that’s unique to you, instead of your Social Security Number. This will help to protect your identity.
